There are two types of oral appliance therapy:

1. Customized oral appliances to align your jaw for the treatment of TMJ Disorders

2. FDA approved oral sleep appliances for the treatment of snoring and Sleep apnea. 

 

TMJ appliances are customized to your specific jaw condition. Your jaw position not only affects the alignment in your TMJ, but also the position of your bite, head, neck, airway, and tongue position. Our doctor makes them small and comfortable so that you can talk in them as they often need to be worn during the day and night. 

 

These appliances are very different from nightguards which are only meant to cover your teeth to reduce wear. Studies and experience have shown that nightguards used improperly can actually worsen a TMJ disorder.  

 

Oral appliance therapy is a safe and successful treatment method for snoring and obstructive sleep apnea. Oral appliance therapy usually involves custom-fit oral sleep appliances that can improve your sleep and help manage your condition.

 

Oral sleep appliances fit like a sports mouth guard or orthodontic retainer and support your jaw in a forward position to assist you in maintaining an open airway.

Our trained doctor conducts a complete clinical evaluation including an examination of your jaw, teeth, tongue, airway, and any necessary testing, such as imaging of the jaw. Imaging is typically a Cone Beam CT or Diagnostic Radiographs. This lets our doctor visualize the anatomical problems in the jaw.

At Cosmo Dental, our doctor creates a personalized treatment plan for you and customizes your oral appliance using physical impressions of your teeth. These models are sent to a dental lab where your appliance is created by a qualified technician.

Facial Pain Q & A

Facial pain can vary depending on the person. You might experience a full and throbbing feeling or an intense, stabbing pain or even burning.

You might experience facial pain due to changes in nerve signaling to the brain. These nerves can become extremely sensitive and the signals going to the brain might not switch off properly.

Pain that starts in the face could be the result of a nerve problem, an infection, or an injury. 

Other causes of facial pain include:

  • Abscessed tooth
  • Cluster headache
  • Herpes zoster (shingles) or herpes simplex (cold sores)
  • Sinusitis
  • Temporomandibular joint disorder (TMJ)
  • Teeth clenching
  • Trigeminal neuralgia
  • Tendon and ligament inflammation. 

Facial pain can also begin in the neck and radiate through the jaw and face.


 

Myofascial pain

Myofascial pain happens when the nerves connected to your muscles become irritated and small nodules form. This causes your muscles to become painful and bunch-up in areas called trigger points. 

Myofascial pain in the jaw muscles can be associated with other symptoms, such as dizziness, eye twitching, ringing in the ears, and tearing. Tension headaches in the temples are typically the result of myofascial pain within the temporalis muscle (the jaw-closing muscle).

 

Trigeminal neuralgia

Trigeminal neuralgia involves sudden attacks of pain on one side of the face. The pain comes and goes suddenly. Some people might be free from pain for many months or years while others experience more frequent flares. This pain can be very severe and debilitating. 

Trigeminal neuralgia is more common with aging and in women, and it’s often caused by impingement in the nerve by blood vessels.

 

Atypical trigeminal neuralgia and chronic regional pain syndromes

These refer to continuous pain that is aching to throb or burning. There can also be a sense of numbness or redness associated with them. They can be a result of some type of nerve injury.

If your TMJ disorder has caused problems with how your teeth fit together you may need treatment such as bite adjustment (equilibration), orthodontics with or without jaw reconstruction, or restorative dental work. Surgical options, such as arthroscopy and open joint repair restructuring, are sometimes needed, but are reserved for severe cases. Our doctor’s do not consider TMJ surgery unless the jaw can’t open, is dislocated and non-reducible, has severe degeneration, or the patient has undergone appliance treatment unsuccessfully.

MINIMALLY INVASIVE TMJ SURGERY

  • Most (roughly 90%) of TMJ disorders involve only the muscles of the mouth and face and not the actual TMJ. These disorders require no surgical treatment and can most often be managed through the following recommendation:
  1. Night Guards and Splints
    • More recent research reveals that splints and night guards are not as helpful as once believed for the treatment of TMD.  However, if you clench or grind your teeth, splints or guards are invaluable resources to protect your teeth from damage while clenching or grinding.
  2. Physical Therapy
    • As with other joints in your body, strengthening the area and improving Range of Motion (ROM) are crucial in the treatment in TMJ disorders.
  3. Home Care
    • Soft Foods
    • Anti-inflammatory medications
      • Naproxen or Ibuprofen
    • Stress Management
  4. Relaxation Techniques
Surgical Treatment
  • Only 10% or less of TMD disorders require surgical intervention.  These disorders typically involve the Temporomandibular Joint itself and not merely the muscles of the jaw and face.
  • When there is displacement of the TMJ disc, damage to the disc, or significant inflammation in the joint then surgical procedures are indicated.
  • Our TMJ Specialist prefers Minimally Invasive TMJ Surgery whenever possible.
  • Avoiding Surgery Whenever Possible 
  • Using surgical procedures that minimize large incisions that result in more scarring, pain, swelling, and recovery. 
  • Minimally Invasive TMJ Surgical Procedures offered at Cosmo Dental:
    • Arthrocentesis and Arthroscopy
      • Surgical procedures avoiding incisions and utilizing only 1-3 needle punctures around your TMJ to irrigate the site, remove harmful inflammation, and improve scarring that may be present. 
  • Arthrocentesis:
    • This technique can be performed in the office under local anesthetic or IV sedation.  It involves the placement of two needles into the TMJ that allow irrigation of the joint and placement of medications only. 

 

  • Arthroscopy: 
    • This technique is similar to arthrocentesis but is an outpatient hospital procedure that involves the placement of slightly larger instruments into the joint.  One of these instruments is a small camera that allows visualization of the joint to better see, diagnose, and treat conditions of the TMJ.  
  • This technique is the main procedure utilized in Minimally invasive TMJ surgery as it allows the best of both worlds-actually being able to see into the joint to better treat disorders while causing minimal trauma and having only tiny puncture sites without incisions
